About the role 

Our Transfer of Care Team is dedicated to managing patient discharges from acute and community hospitals across Cambridgeshire. We pride ourselves on a Home First approach, ensuring individuals can stay within their own communities while receiving the care and support they need.

You won’t need to have a social work qualification for this role but knowledge, experience and understanding of the needs of vulnerable adults is essential. Working knowledge and experience of the Care Act, Mental Capacity Act and Cambridgeshire County Council’s Adult Social Care policies and procedures will also be an advantage, particularly around wellbeing and outcomes. Training and a support will be provided to the successful candidate.

About you 

You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries. 

It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative. Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience. 

You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Experience of multi-disciplinary working to enable best outcomes for individuals.
