Band 6 Physiotherapist – Sheppey Community Hospital
We’re looking for an experienced and enthusiastic Band 6 Physiotherapist to join our exciting Rapid Response and Intermediate Care Team to assess and treat patients within the local community. The Physiotherapist would be working closely with Physiotherapy colleagues as well as working collaboratively with our Occupational Therapists, Nurses and Support Worker colleagues.
The service operates 7 days a week 8am till 8pm, and so includes weekend and bank holiday working.
The team is headed by the Clinical Service Manager for Swale Rapid Response and Intermediate Care, and has 3x Clinical leads in post for nursing, physiotherapy, and occupational therapy. They provide line management and clinical support to the team.
Main Responsibilities
Within this role, you will:
- Work within Rapid Response and Intermediate Care across the Swale locality
- Complete complex holistic initial assessments with patients with a wide range of physical conditions
- Triage patient referrals from a variety of sources
- Liaise with family members, carers, and wider support services
- Devise and review care plans
- Delegate rehabilitation programmes to support worker colleagues
- Complete follow up visits with patients
- Attend and participate in clinical meetings, team meetings, service development workstreams.
- Provide training and clinical support to colleagues
- Offer and support student placements from local universities
- Supervise and appraise staff within your line management.
Ideal Candidate
This is an opportunity to put your Degree in Physiotherapy and previous work experience to great use. You will have a full driving license with access to a car, you will be motivating, innovative and adaptable, with the ability to work flexibly across specialities as need arises, holding the HCPC (Physiotherapist) registration. With strong communication and organisation skills, and a flexible approach you will ensure delivery of positive experiences to patients at every stage of their healthcare journey. You will also take an active role in supporting and developing colleagues through sharing of knowledge and experience and within the formal supervision and appraisal processes.
We are also able to support any return to practice therapists, in line with the CSP and HCP return to practice guidance and registration process.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
