How you’ll make a difference
As a Social Care Practitioner (Social Work), you will provide support and promote independence for vulnerable adults in our community, assisting them in their time of need.
What you will be doing
- It will be your responsibility to assist in organising and providing effective care services, this may include the assessment and review of care plans.Â
- Working under the direction of Social Workers, you will contribute to and review plans and services to improve intervention ideas.Â
- To protect vulnerable people, you will refer issues in accordance with safeguarding procedures, this will require you to maintain confidential reports to professional and council guidelines.
- It will be key for you to effectively communicate with service users and providers, providing advice and guidance when necessary.
- Daily, you will be working with Social Workers and external contacts, such as education services and health agencies, ensuring strong working relationships are achieved.
What we need from you
- We require you to have experience in a relevant area of statuary or voluntary sector work in health and social care.
- You will demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.Â
- You will be able to gather information from a variety of sources and follow the assessment process. Â
- You must have the ability to communicate effectively with services users and other professionals so you can promote independence and personal choice, whilst understanding service users needs.
- Ideally, you will hold a relevant qualification e.g. NVQ Level 3 in a health or social care field. Â
Additionally, to be considered as an Advanced Social Care Practitioner:
- You must have a minimum of 2 years experience in a relevant area of statutory or voluntary sector work in the health or social care field.
- We require you to have a relevant qualification, eg NVQ Level 4 in health, social care or equivalent.
What you need to know
- To be considered for the Advanced Social Care Practitioner position, your evidence must be clearly detailed in your application. The salary is offered at points within these bands depending on experience:
- Social Care Practitioner: £29,540 – £32,061
- Advanced Social Care Practitioner: £32,597 – £35,412
- This role is hybrid, with a mixture of working at our office in Badminton Road, Yate and working occasionally at home.
- You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
- Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
